Description: Medicine for Seniors

 Specialty services offered for people over 60 years of age; patients with chronic diseases and multiple diseases; patients with signs of dementia or Alzheimer’s disease.

 These services include: Examination, screenings, follow-up and referral services as well as  Social Services eligibility for a range of programs.

 Specialty care for seniors and patients with chronic disease provided by an experienced Geriatrician. The program offers a team approach with input from the Geriatrician, Primary Care provider, medical assistant and social worker to resolve the unique issues that Senior patients present.

Hours: First and third Mondays of the month from 1:30 to 5:30 p.m.
